Since 1992, On Track has developed a range of support programs which have assisted individuals to achieve their personal goals. Our aim is to improve an individual's quality of life and assist them to participate more fully within the local community. On Track is committed to working with families, carers, GPs, allied health professionals and other service providers to meet the needs of each client. Our aim is to increase the opportunity for each person to participate more widely within their community.

While we play an important role in the delivery of quality support services to people across the North Coast region, successful outcomes for our clients and consumers can only be achieved by forging and maintaining strong partnerships with other services. Our vision statement reflects this commitment : "Connecting people to their communities".

Currently our range of programs includes:

  • Disability Services - Supported Accommodation, Attendant Care Packages, Leaving Care Program, Criminal Justice Program, Disability Housing and Support Initiative (DHASI), Carer Respite, Community Access and the NSW Younger People in Residential Aged Care (YPIRAC) Program.
  • Disability Employment Services - Transition to Work, DEN Open Employment and ADE Business Service supported employment
  • Mental Health Services - Housing and Support Initiative (HASI), Supported Accommodation, Crisis Accommodation, Residential Rehabilitation, Resource and Recovery Programs, Day to Day Living Program.
