Outreach Service

The On Track Outreach Service provides a range of accommodation and psychosocial support services for people living with a mental illness in the community. On Track Community Programs properties are leased to individuals and families.

Outreach workers are available Monday to Friday between business hours. Outside of these hours, consumers can also contact support staff on duty when urgent.

Outreach workers provide support through a case-management framework incorporating other community-based and government agencies.

On Track Community Services -

The Outreach Service has co-case management plans and liaises with Community Mental Health case managers, Department of Community Services, Department of Housing, drug and alcohol counselors, Bugalwena Service, NSW Carers Association, as well as local general practitioners and psychiatrists and psychologists to achieve the best outcome for our consumers.

Consumers are encouraged to participate in "life skills" training with the goal of eventually returning to private independent accommodation when appropriate.

Entry to the Outreach Service is through referral by Community Mental Health or the Tweed Valley Clinic - the psychiatric inpatient unit. Consumers are initially accommodated around the home while a transitional plan is prepared to assist individuals to access our supported accommodation outreach service.
