Supported Accommodation

On Track Community Programs provides a range of supported accommodation options within its mental health program.

One of these options are hostel-style housing for people living with mental illness who have high support needs. People who enter this style of accommodation will be referred via the In Patient unit of Community Mental Health. Consumers are assisted with rehabilitation so that they can move to independent living in either private or public housing or to one of the On Track Outreach properties. This environment provides a setting that supports residents to develop life skills, and assists with such things as attendance at medical appointments, budgeting and personal development.

On Track Community Services -

All residents are encouraged to reach their individual goals to gaining of confidence and self esteem which assists them towards independent living and community based involvement.

On Track is a transitional style of support but is not used for crisis accommodation.

Respite options can be provided at On Track and this is for people who are referred via their community health case manager for a planned occasion of respite or within On Track's outreach properties. Respite is provided to consumers who have carers or family members who require some time out.
