Leaving Care
On Track's Leaving Care Program supports young people with a disability to transition from the care of the Minister for Community Services to live as independently as possible. The program provides transition planning, case management and access to support services that are linked to accommodation, vocational opportunities and skill development as well as opportunities for social and community participation.
The program is focused on providing support services for young people who:
- are unable to return to their birth family
- where the young person has had significant casework and service interventions to restore them into the family home without success
- or are unable to remain with the foster family, without further assistance
The Leaving Care Program is sponsored by the Department of Ageing Disability and Home Care (DADHC) and underpinned by partnerships with the Department of Community Services (DoCs) and the Department of Housing (DOH).
